भुगतान करने के बाद लेन-देन संख्या (UTR / UPI संख्या) संभालकर रखें — आवेदन में वही भरनी होगी। / After paying, keep the transaction ID (UTR / UPI reference). The application form asks for it.

आवेदन कैसे करें / How to apply

1. 1 मोबाइल नंबर दर्ज करें और एसएमएस से आए ओटीपी से उसे सत्यापित करें। / Enter your mobile number and verify it with the OTP sent by SMS.
2. 2 आवेदक, संपर्क और आय का विवरण भरें। / Fill in your applicant, contact and income details.
3. 3 आवेदन शुल्क भेजें और लेन-देन का विवरण फ़ॉर्म में भरें। / Send the application fee and enter the transaction details in the form.
4. 4 नीचे दिए गए दस्तावेज़ अपलोड करें। / Upload the documents listed below.
5. 5 पूरा आवेदन एक बार जाँचें और जमा करें। / Check the whole application once and submit it.
6. 6 रसीद का लिंक संभालकर रखें — वही आपका आवेदन रिकॉर्ड है। / Keep the receipt link safe — it is the record of your application.

फ़ॉर्म बीच में छोड़ा जा सकता है; उसी मोबाइल नंबर से वापस आकर वहीं से जारी रखें। / You can leave the form part-way and come back to it with the same mobile number.

• आयकर रिटर्न (कम्प्यूटेशन सहित) या आय प्रमाण पत्र / ITR (with Computation) or Income Certificate अनिवार्य / Required
• आरक्षित वर्ग प्रमाण पत्र / Reserve Category Certificate आरक्षित हेतु अनिवार्य / Required if reserved
• स्वप्रमाणित घोषणा पत्र / Self-Declaration Form अनिवार्य / Required
• बैंक विवरण (पासबुक या कैंसिल चेक) / Bank details (passbook or cancelled cheque) अनिवार्य / Required

• अनिवार्य दस्तावेज़ों के बिना आवेदन जमा नहीं होगा; वैकल्पिक छोड़े जा सकते हैं। / Required documents must be uploaded before you can submit; optional ones can be left out.
• हर फ़ाइल पीडीएफ़ या फोटो (JPG, PNG), अधिकतम 5 एमबी — मोबाइल की फोटो भी चलेगी। / Each file is a PDF or a photo (JPG, PNG), up to 5 MB — a phone photo is fine.
